    We are delighted to bring you this latest edition of the Schema Therapy Bulletin (STB)"Integrating Concepts from Other Therapeutic Models with Schema Therapy."

    Schema therapy is inherently an integrative model. It was developed by Dr. Jeffrey Young by integrating theory and techniques from a range of models, including psychodynamic, object-relations, cognitive, behavioral, interpersonal, Gestalt techniques, and more. This has provided us with a therapeutic approach informed by developmental and attachment theories and enriched by an amazing array of techniques across experiential, cognitive, behavioral, and interpersonal realms.

     

    In this ‘bumper issue’ of the STB, we have included ten articles that describe techniques and theoretical concepts from other therapeutic models that may enhance our understanding and practice for working with clients primarily with a schema therapy foundation.
     

    Felicity Nichols in her article, “Holding Mind in Mind: Mentalization-Based Treatment and Schema Therapy,” explores how concepts from mentalization-based therapy (MBT) can be interwoven through schema therapy. Mentalization is a psychological process which allows us to make sense of behaviors through understanding our own thoughts and feelings. Felicity describes how a capacity for ‘good mentalizing’ can overlap with our understanding of Healthy Adult mode traits, and ways in which MBT may complement our work in schema therapy.   
     

    Harriet Achtentuch in her article, “Integration of EMDR concepts and techniques in Schema Therapy,” explores eye movement desensitization reprocessing therapy (EMDR), and how it can complement schema therapy through strengthening the Healthy Adult mode and enhancing trauma processing.
     

    Alp Karaosmanoglu and Bahar Kose Karaca in their article, “Reviewing Schema Therapy Concepts through the Frame of Polyvagal Theory,” explore ways in which polyvagal theory can enhance our conceptualization of schema coping modes. They describe ongoing research which is exploring ways in which our understanding of the workings of the nervous system enables us to understand mammalian coping mechanisms more deeply.
     

    Eckhard Roediger and Rob Brockman present, “Integrating the functional ACT-processes into the Schema Therapy model." In this article they explore ways in which acceptance and commitment therapy (ACT) processes can be integrated into schema therapy. They describe how ACT can help us to understand Healthy Adult functioning in greater depth, and how it can provide useful ideas regarding how to strengthen this mode. They also describe ways in which ACT can enable us as therapists to stay out of our own coping modes and take a flexible approach in order to facilitate client autonomy. 
     

    Lissa Parsonnet and Robin Spiro share their article on, "Integration of ideas and concepts from Internal Family Systems Therapy (IFS) with Schema Therapy." They highlight ways in which IFS works with ‘parts’ (equivalent to modes in schema therapy terms), and provide some fascinating insights into working creatively with strong affect in Imagery Rescripting.
     

    Agnes Sullivan presents,"Integration of Emotionally-Focused Therapy (EFT) Skills into Schema Therapy." She describes how an understanding of the EFT model can enhance ways in which schema therapists conceptualize and apply limited reparenting and attunement. A case study is provided which brings these ideas to life, showing how bonding and stabilization in schema therapy can be augmented by EFT techniques.
     

    Chris Hepworth presents, “Methods To Manage Emotions – DBT and Emotional Regulation Skills in a Schema Therapy Framework." In this article, he shows why it can be helpful to teach clients skills in emotional regulation, distress tolerance, interpersonal skills, and mindfulness in the context of schema therapy, on the basis of their individual conceptualization and unmet emotional needs. He provides guidance on specific skills which can be taught to clients to facilitate Healthy Adult coping within a schema therapy context.
     

    Limor Navot writes, “Looking at the Bright Side: the Healthy Adult Strengths Model," focused on integrating positive psychology with schema therapy. She describes a new model of the Healthy Adult (Bernstein, in preparation) based on the concept of the healthy part of the personality as an adaptive system designed to fulfill universal life tasks, including identity, attachment and negotiating social hierarchies. She illustrates how positive psychology and a strengths approach can enhance our understanding of Healthy Adult functioning in schema therapy.
     

    Janis Briedis and Susan Simpson present, “Integration of Schema Therapy and Comprehensive Resource Model – Expanding Ways of Working with Highly Traumatized and Dissociated Clients.” They describe how the comprehensive resource model (CRM) trauma processing approach can enhance the client’s capacity to remain embodied during trauma processing. Further they suggest that CRM provides powerful mechanisms for enhancing growth of the Healthy Adult self.  They provide case studies, illustrating ways in which CRM can enhance experiential schema therapy work both in individual and group contexts. 
     

    Galit Galid presents a third part in her series on working with Infant Modes (see previous editions of the STB for Parts 1 and 2). In part 3, she draws on the psychoanalytic infant research of  Daniel Stern to deepen our understanding of the universal needs and common deficits of infants. Galit also shares the unique way that experiences are registered at this age which may be later manifested in adult therapy.

    Gene-Environment Interactions in Depression

    Have you seen the fascinating presentation at the 2020 ISST Virtual Summit investigating the genetic and environmental influences leading to depression?

    Sarah Cohen Woods shares some of the latest research that helps answer some of these influences, as well as posting more intriguing questions during her keynote address at the 2020 ISST Virtual Summit.

    Sarah discusses how a large number of biological and molecular perturbations associated with psychological disorders have been identified, including changes in brain structure, neurological connections, inflammation, and epigenetic factors, as well as genomic risk.

    However, integration with how these factors relate to the development and maintenance of schemas, critical in psychological disorders, is still emerging.

    Genomic risk, and potentially dynamic biologic and neurological changes, need to be integrated with current perspectives of schemas, and the role of early experiences, such as trauma.

    Sarah describes neurobiological processing variations related to schema-related learning, differentiating between specific stages of schema development. Genomic risk factors for psychological disorders that exhibit early maladaptive schemas, such as depression, are being identified.

    Sarah shares the latest research identifying gaps and new pathways forward in the investigation of the neurobiological bases of schemas, and how improving the neurobiological understanding of schemas may benefit clinicians moving forward.

    We hope that everyone enjoyed the amazing 2020 ISST Virtual Summit that took place on May 28 - 30th! There was a fantastic line-up of virtual workshops and presentations, on a range of topics.

    A real highlight was the appearance of Dr. Jeffrey Young, who presented a keynote interview along with a memorable question and answer session. In addition, Dr. Young co-presented a workshop selection.

    The whole event was a real credit to the conference committee, spearheaded by Travis Atkinson, who put an enormous amount of work in a short amount of time to create a digital platform, the Schema Summit App on iOS and Android devices and browsers, along with organizing the presenters and caring for the numerous technical aspects involved in a virtual summit.

    We also thank all of the incredible presenters of the summit for their hard work putting together virtual presentations, and the gifts of their knowledge and skills they shared with participants.

Cognitive interventions to help patients form more positive and stable friendships," describes the mechanisms that contribute to ambivalence in friendships and the role of maladaptive schemas and modes in intensifying these patterns.  She describes the 'Friendship Flower,'  a cognitive tool to help adolescents and adults to explore and enhance friendships.

    Christof Loose and Maria Galimzyanova, in their article, "Schema Therapy Focused on Parent-Child Relationships,"explore the influence of schemas and modes within parent-child relationships. They make recommendations on how to best enhance these relationships with the help of schema therapy.

    Wendy Behary and Michael Watkins, in their article, "Why Leaders Do the Dysfunctional Things They Do," explore the way in which maladaptive schemas and modes can undermine the effectiveness of those in leadership roles. They suggest new ways organizational relationships can be enhanced through the application of schema therapy principles.

    Mary Giuffra, in her article, "Healing Relationships with Traumatized Modes," explores the role of trauma in driving divisions and disconnection in relationships. She provides guidance on healing trauma-driven schemas.

    Jeff Conway and Manolya Aydagul, in their article, "The Enmeshment and Undeveloped Self Schema and Conflict in Non-Romantic Relationships," explore how this schema can lead to conflict avoidance, and interfere with intimacy and expression of the authentic self in the content of non-intimate relationships.

    Finally, Vivian Francesco interviews former ISST Executive Board President, Wendy Behary.

    Group Schema Therapy for BPD


    How do we measure the success of group schema therapy for borderline personality disorder (BPD), and how does it compare to individual schema therapy for BPD, along with specialized treatment as usual for BPD?

    Arnoud Arntz and Joan Farrell present their keynote addresses at the 2020 ISST Virtual Summit.

    Arnoud discusses the results of a three year international multi-center trial of nearly 500 participants. He shares details about the three comparison conditions that were used for the study:

    1. group schema therapy sessions, with a limited number of individual schema therapy sessions

    2. half group schema therapy sessions, and half individual schema therapy sessions

    3. specialized treatment as usual for BPD based on the site

    Arnoud reviews how results were measured on several factors, including treatment retention, the primary outcome of BPD-severity over three years, the impact of each modality on the most common symptoms of BPD, among others. Watch as he presents the fascinating results to see which group fares best.


    Joan shares the clinical implications of the trial's outcomes, along with her work from research over 10 years in group schema therapy since she presented impressive results along with Ida Shaw at the first ISST conference in Coimbra, Portugal in 2008.  

    Joan breaks down how to account for different levels of treatment and correlated results, including the competency and training protocols of therapists, and how these factors affected patient outcomes on the BPDSI. 

    Joan shares some important caveats that are needed when conducting group schema therapy, including specific challenges inherent in a group modality. She answers the question of whether every therapist can be an effective group schema therapist.

    Joan also shares how patients with narcissistic personality disorder (NPD) fared in groups for BPD, and which schemas can best be treated in group schema therapy, among many other crucial aspects involved.

    Watch the ISST Webinar: A Phase-Oriented Approach to Schema Therapy

    Schema therapy is a complex model consisting of a wide variety of techniques aimed at treating populations with a variety of challenges.

    The task of the schema therapist is to connect with clients, to help them identify the love, stability, and security they may have lacked in their childhood, and to teach them how to remedy those losses.

    As an intense form of psychotherapy, practitioners of schema therapy often ask many questions about how to best intervene, including:

     

    • Where do you start in treating clients?
    • How do you proceed with treatment?
    • How do you help clients experience the depth needed for change to occur while still practicing efficiently?
    • When do you conclude treatment with different populations?


    Rosi shares guidance and advice on how schema therapists can separate treatment into distinct phases, each with its own framework, goal, approach, and exercises.

    Participants will learn about the characteristics of each phase, and become familiar with specific interventions linked to each phase.

    The goals of this interactive webinar are:

    1. Develop a practical guide to using the phase-oriented method in schema therapy

    2. Learn guidelines to help a sometimes complex therapy process become more transparent

    3. Experience exercises and tips from clinical practice that illustrate the phase-oriented approach to schema therapy

     

    About the Presenter

    Rosi Reubsaet is a registered clinical psychologist in the Netherlands who specializes in treating people suffering from severe personality disorders. She is co-founder of the Academy for Schema Therapy, which includes delivering trainings in schema therapy and the phase-oriented method. In addition, Rosi is co-founder of the House for Schema Therapy, a treatment facility for patients in the Netherlands.

    Rosi is author of the book, "Shematherapie: werken met fases in de klinishe praktijk, " (Houten: Bohn Stafleu van Loghum, 2018). She is also the author of the forthcoming book, "Schema Therapy: A Phase-Oriented Approach for Clinical Practice," (Pavilion Books, 2020).

    Rosi is an ISST-Approved Advanced Certified Schema Therapist and Supervisor/Trainer, and a certified supervisor by the Dutch Schema Therapy Association. In addition, she is registered as a Cognitive Behaviour Therapy Supervisor. Rosi served as a Board Member for the Dutch Schema Therapy Association from 2012 to 2018. Currently, she is a Board Member for the Personality Disorders Section for the Dutch Association for Cognitive Behavior Therapy.

    Imagery Rescripting


    Imagery Rescripting is a powerful change technique of schema therapy. At the core of the intervention are methods to form greater therapeutic bonds with clients, along with opportunities to create corrective emotional experiences between the client and the therapist.

    Understandably, therapists face several challenges when conducting Imagery Rescripting with their clients. For instance, clients can present difficult circumstances within the imagery, and therapists may not always be prepared for how to address unforeseen issues. The experience of the therapist, combined with their skill and knowledge levels, can strongly influence the effectiveness of the technique.

    Remco and Chris share specific techniques during the workshop to help schema therapists build skills while conducting Imagery Rescripting, including: 
    • how to help clients strengthen their Healthy Adult 
    • how to manage antagonists that come up within the imagery
    • how to address atypical situations, including troublesome responses clients can sometimes experience
    • how to help clients confront antagonists within the imagery
    • how to help clients increase their tolerance for more intense emotions
    • how to apply the latest advances in Imagery Rescripting research

    Nick was a member of "The Tenors," based in Canada. He has worked with Jorge Calandrelli, famed producer, music arranger, and songwriter for artists including Josh Groban, Andrea BocelliPacido Domingo, Tony Bennett, Barbara StreisandIl Divo, and Celine Dion. 

    Nick starred as Joseph in Andrew Lloyd Webber's Off-Broadway musical tour, "Joseph and the Amazing Technicolor Dreamcoat."

    About Nick

    Raised in Los Angeles, California, from a musician father and mother whose family built pipe organs,  Nick attended the Southern California Conservatory of Music to pursue a career as a professional studio. musician. He earned a BA in Voice and Classical Guitar, and then trained with Placido Domingo's mentor for 7 years.

    Nick works with accompaniment, ranging from a pianist, strings, 5-piece bands, all the way up to full orchestra or prerecorded tracks.


    His repertoire spans over 200 songs and arias in nearly every genre.

    Nick regularly headlines at performing arts centers, along with venues including the VenetianPlanet Hollywood, and Caesar's Palace in Las Vegas, Nevada.

    Nick often performs at charity events, including a recent Breast Cancer Awareness concert fundraiser.

    Carly & the King, also known as Carly and Sebastian, met in songwriting class at Berley College of Music in late 2013. They embarked on a two-year adventure traveling around the world, performing to audiences on Celebrity Cruise Lines throughout Northern Europe, the British Isles, and the Caribbean.

    Since anchoring in their hometown of Los Angeles, California, they have performed at renowned establishments, including the House of Blues, the Hotel Cafe in Hollywood, the Staples CenterDowntown Disney, and Rockwood Music Hall, along with resorts such as the MontageSt. Regis and the Ritz Carlton. 

    Clients have included Disney, Citi National Bank, QSC, and L'oreal.

    Stemming from the Seattle, Washington music scene, Patrick Sieben has quickly become one of the top performers in Las Vegas, Nevada, performing at Aria Resort and at Mandalay Bay, where he currently has residency, entertaining in the heart of the Las Vegas Strip.

    Patrick & the LVB capture a young and energetic mix of modern pop tunes, classic hits and original music.

    While maintaining his residency in Las Vegas, Patrick and his band have also traveled extensively, sharing their talent with audiences around the world.

    Patrick opened for TOTO and YES on multiple dates, including at the Greek Theatre in Los Angeles, California, and at Barclay's Center in Brooklyn, New York.
